Personality disorders result when an individual has unhealthy and rigid thinking patterns. I selected borderline personality disorder (BPD) among the cluster-B personality disorders. The etiology of BPD includes genetics and environmental factors such as childhood trauma, poor maternal attachment, sexual or physical abuse, and emotional neglect. BPD. The DSM V diagnostic criteria mainly present with hypersensitivity to rejection. As a result, an individual is not stable in behavior by trying to avoid imagining or real abandonment, self-image, affect marked by anxiety, episodic dysphoria or irritability, and unstable relationships, which alternate between devaluation and idealization (Kulacaoglu & Kose, 2018).They can also be impulsive in areas that can be damaging, including; substance abuse and reckless driving, among others. An individual with BPD can also have a chronic feeling of emptiness and intense anger. BPD causes distress and impairment in social and occupational life.

Therapeutic approach

The best approach for managing borderline personality disorder is dialectical behavioral therapy. DBT is a type of cognitive behavioral therapy that aims to treat the various dysfunctional behaviors resulting from mental health problems. Initially, it was purposely for suicidal individuals and those with BPD, but recently, it has been used for all individuals who experience difficulty managing their emotions .Through DBT, individuals with BPD can adopt various healthy ways of dealing with distress, regulate emotions, have good interpersonal relationships, and be mindful. DBT helps individuals with BPD to acquire those skills that can help them deal with various distresses.

The patient and a therapist take part in DBT  for 8 to 12 months. It involves four elements: individual therapy, group therapy, phone calls, and weekly consultation meetings. Individual therapy focuses on improving the quality of life by restricting an individual cognition, and the therapist and the patient meet weekly for the sessions. Group therapy focuses on training the patients on skills that can help them deal with difficult situations. The phone calls help apply the skills learned during the therapy sessions. During the consultation meetings, the therapist focuses on supporting the patient and motivating them to adhere to the therapy (Rudge et al., 2017). Therefore, DBT is an effective therapy for BPD.

Therapeutic relationships

Therapeutic relationships are interactive relationships between the patient and the therapist that focus on achieving specific goals. A good therapeutic relationship in healthcare enhances patient satisfaction, improved health outcomes, decreased rehospitalization rate, and decreased hospital duration (Bolsinger et al., 2020). A positive environment during BPD therapy in any setting, including family, individual, or group, is related to positive outcomes. During the therapeutic relationship, I would ensure that I listen actively to the patient’s problems and understand their feelings. Through this, I would ensure that the patient focuses on the present facts, and in cases where their emotions are distracted, I would want to know the cause and find the best ways to help them. In case of a group session, I would adapt the group members’ various ideas and solutions and educate them on the need to take the group and family meetings as a safe setting for solving their issues and achieving the goals set.
