Introduction

The work of the Robert Wood Foundation Committee Initiative that led to the IOM report, “Future of Nursing: Leading Change, Advancing Health”
The IOM report was produced following a joint effort between the Institute of Medicine (currently The National Academy of Medicine) and The Robert Wood Johnson Foundation. In 2008, the two organizations launched a two-year initiative to emphasize the necessity to evaluate and change the profession of nursing. The RWJF committee was given the responsibility of whether the nursing workforce was capable of meeting the demands of a reformed health care system. The committee was to develop recommendations, including those addressing the ability of the education system to produce well-prepared nurses and the provision of nursing services amid nursing shortages. The committee was to define a blueprint and agenda for action, entailing alteration in institutional, and public policies at the local, state, and national levels (Rekha, 2021).
The core duty of the committee was to perform an assessment and give recommendations related to several issues, with aim of recognizing nurses’ crucial role in the design and implementation of a health care system that is more efficient and effective. These four messages have greatly influenced nursing education and training, nursing practice, nursing workforce development, and nursing leadership. Many states have amended their laws to grant APRNs full practice authority. For example, since 2010, nine states (Vermont, South Dakota, Connecticut, Nevada, Minnesota, Maryland, Rhode Island, North Dakota, and Nebraska) amended their scope of practice laws to allow APRNs to practice independently. At present, twenty-two states and the District of Columbia have granted APRNs full practice authority, meaning that these health professionals can practice without the requirement for physician collaboration/ supervision in 22 states and the District of Columbia. Also, the messages have made more nurses attain a baccalaureate degree. According to the American Association of Colleges of Nursing (2021), around 56 percent of registered nurses have a Bachelor of Science in nursing or higher degree, up from 49 percent in 2010 when the report was released. Nurse practitioners transition to practice residency programs operate in numerous healthcare settings, for example, federally qualified health centers, retail clinics, hospitals Department of Veteran Affairs primary care centers, Today, registered nurses are working as part of a multidisciplinary team with health professionals who holds a doctorate or master’s degree. The role of state-based action coalitions and how the coalitions help advance the goals specified in the IOM report
State-based action coalitions perform the work of the campaign at the state, regional, and local levels. According to Yoder-Wise (2018), state action coalitions are working with the American Association of Retired Persons (AARP) and the RWJF imitative known as “The Future of Nursing Campaign for Action”. The action coalitions are working with universities and colleges, funding sources, nursing organizations, and state government agencies including state nursing boards. The state coalitions are striving to ensure nurses can advance their education t and take on advanced nursing roles and be able to shape the reformed health system.
Two initiatives spearheaded by Texas action coalition and the ways the initiatives advance the nursing profession.
Texas action coalition is working to transform health care and nursing in Texas and build healthier communities. The Texas Team believes that all Texas residents deserve better care, lower costs of health care, and better health. As the largest segment of health professionals in the state, nurses are necessary for attaining the goals in Texas. Texas Team is working to remove SOP practice barriers and ensure nurses engage in lifelong learning. These initiatives advance the nursing profession because the removal of SOP restrictions will enable nurse practitioners to practice to the full level of their education and training and nurses will regularly update their skills to be able to resolve issues they might encounter while providing patient care.
Barriers to advancement that exist in Texas
Restrictive scope of practice laws are barriers to advancement that presently exists in Texas. Under the current law, APRNs are required to have a delegation agreement with a physician for them to conduct medical acts such as performing diagnosis, ordering, and evaluating diagnostic testing, and prescribing medications. According to Phillips (2021), in Texas, the authority of APRNs to make a medication diagnosis and prescribe medications should be delegated by a medical doctor utilizing written protocols along with a prescriptive authority agreement that outlines the devices and drugs that the APRN might prescribe or order. The delegating the physician and the APRN jointly develop and sign the protocols. Nursing advocates in Texas overcome these barriers by advocating for amendment of the current laws to grant APRNs the ability to perform medical tasks without physician collaboration.
