Practice Problem

The practice problem idea is the use of the Patient Health Questionnaire (PHQ-9) tool in the diagnosis of major depressive disorder. The device is a 9-item, self-administered screening tool useful in the diagnosis and tracking of symptoms of patients with MDD. It helps in tracking the overall severity of depression in patients as well as tracking some specific improvements of depressive symptoms for patients on treatment (Sun, et.al., 2020). A diagnosis of depression is made if patients exhibit five out of the 9 symptoms for more than half of the days during the past two weeks (Muñoz-Navarro et al., 2017). The tool, therefore, is beneficial to millions of Americans at risk for developing depression as they are able to conduct self-assessment tests to determine depressive symptoms. Healthcare providers are also able to screen for and diagnose depression early enough before complications arise. This provides an improvement in quality care and improves health outcomes.             Interprofessional collaboration has been recognized by leading healthcare organizations including World Health Organization as a major necessity for ensuring quality healthcare. Advanced practicing nurses should therefore be armed with competencies in foundations of transformational leadership to ensure collaboration with other professionals (Goldsberry, 2018). As such, the nurses need to have knowledge and skills in the four leadership domains for advanced practice. These include knowledge in clinical leadership, professional leadership, leadership in health systems, and health policy leadership. Each of these domains requires a specific set of skills for effective management.

To ensure proper use of interprofessional collaboration for change implementation, I will adopt the practice of effective communication for quality care provision and improvement of patient outcomes. Team-based care requires interaction and consultations with different practitioners from a wide range of specialties including doctors, nurses, pharmacists, and physical therapists among others. Ineffective communication in such cases can lead to care overlap and misdiagnosis of patients (Busari et al., 2017). Adopting a practice of effective communication leads to a reduction of redundancy and error, ensuring quality outcomes and patient safety.

Another aspect to explore in interprofessional collaboration for change implementation is standardization. Ensuring a uniform process of sharing and upholding procedures is vital for patient safety (Reeves et al., 2017). It ensures clarity in the process of diagnosing and treating patients, avoids confusion related to diagnosis and prescriptions, reducing medication errors, inefficiency, and loss of motivation among healthcare professionals.

Working within a continuum of care will also ensure quality care provision. Healthcare is provided to patients across the care continuum which includes hospitals, ambulatory care, long-term care, schools, and home-based care. In such cases, interprofessional collaboration ensures excellence in the roles played by providers in giving the patients unique, patient-oriented treatment plans. Consultations across the multidisciplinary team involved in direct and indirect care of patients ensure that key aspects of the patient’s concerns are highlighted and relevant care implementation made for the patients.
